7. Upon termination of employment, an employee may provide evidence of termination and elect to request
his/her Hour Bank credits to be refunded, or continue his/her coverage until credits are depleted.
Break in Service
If an employee ceases to be eligible because the hours of work credit fall below the required hours necessary to remain
eligible for coverage, the break in service will be handled as follows:
1. The employee will be considered on leave of absence during the first 90 days and may have his/her coverage
reinstated during this period if he/she again meets all eligility requirements.
2. If an employee again becomes eligible for benefits after a 90-day period, he/she will be considered a new
employee and a new effective date will be established
